In-home testing is a good option for patients who can’t make it to the lab frequently, who travel often, who have difficulty leaving their homes, or who want to be more independent. Whatever your reason for switching to home testing, the key is that you remain committed to testing your levels. “The testing machines themselves only have two buttons and are pretty easy to use.
NATF is dedicated to improving the lives of those affected by blood clots and related diseases. Through our comprehensive resources and innovative programming, we strive to educate patients and healthcare providers about thrombosis and its complications. The goal of home PT monitoring is to help the individual maintain an INR in the desired therapeutic range. International Normalized Ratio is a system established by the World Health Organization and the International Committee on Thrombosis and Hemostasis for reporting the results of blood coagulation tests. The INR is the ratio of the individual's prothrombin time compared to the mean prothrombin time for a group of normal individuals.
WHY A HOME PT/INR MACHINE?
Warfarin must be monitored to ensure it is working effectively and being used safely. Achieving the correct warfarin dosage can be difficult but is extremely important. If the dose of warfarin is too low, the patient is at risk of developing harmful blood clots. If the dose of warfarin is too high, the patient may be at risk of serious bleeding. INR test meters may be prescribed to patients for use in the home, and they may also be used by health care providers at the point of care, such as a health care setting.
The ProTime® Microcoagulation System for prothrombin time testing is designed to safely monitor warfarin therapy. The ProTime System consists of the ProTime instrument, the reagent cuvette with built-in quality control, and the Tenderlett® Plus sample collection system. The test procedure is simple, and performance is comparable to laboratories. Clinical Policy Bulletins are developed by Aetna to assist in administering plan benefits and constitute neither offers of coverage nor medical advice. This Clinical Policy Bulletin contains only a partial, general description of plan or program benefits and does not constitute a contract.
Home INR Testing and Monitoring Helps Manage Conditions Including:
In the case of BioTel Heart they are in California so they needed to call BCBS of California who would then work with BCBS of texas. BCBSTX cannot apparently work with a provider directly that is not registered with the state of Tx. I'm honestly surprised that BioTel heart agents didn't appear to know this and they had to hear it from me. The machine is simple, has built in quality controls, no special maintenance required. In this busy world many patients lack time, are elderly or unable to travel to a lab to be tested.
Hospitals, nursing homes, coagulation clinics, and outpatient facilities should follow the reporting procedures established by their facilities. INR test meter users should also report these problems to the manufacturer using the phone number listed on your meter’s instructions. Alternatives to home PT monitoring include visits to a laboratory facility or health care provider’s office for drawing and testing of a venous blood sample. Physician consultation is advised to make an informed decision based on an individual’s health needs.
What is Warfarin?
“We have policies in place to help with that.” Patients who self-test must also understand that they may have to return to the clinic for testing if their INR levels are found to be high. “I think a lot of patients don’t think of home testing as an option because they don’t really know about it,” explained Dr. Peter Collins, an Advanced Clinical Practice Pharmacist at Brigham and Women’s Hospital . Dr. Collins works in the anticoagulation clinic at BWH and helps patients on warfarin get set up with in-home INR testing. The patient must undergo an educational program on anticoagulation management and demonstrated the correct use of the device prior to its use in the home.
Most of the people I spoke to I swear were in their first week on the job asking if I needed home health or to rent equipment like a wheel chair.. A federal government website managed and paid for by the U.S. Before sharing sensitive information, make sure you’re on a federal government site. It also only applies to people with stable hematocrit within the acceptable fingerstick testing range, so they must have been screened for anemia first. Care Plans Guide
A test strip is inserted into the meter, and then a lancet is used to obtain blood which is applied to the test strip. The meter reads the test strip, measures how long it takes the blood to clot, and provides the result based on a standardized calculation in the form of the International Normalized Ratio, or INR. The INR is used by patients and physicians to determine if warfarin dosing is appropriate.
The company basically lets us use the machines for "free" but we have to buy the test strips through them...which are like $6 a strip. Insurance coverage for home testing varies from plan to plan. Many insurers cover the cost of home testing, but some patients may have a copay or some out-of-pocket expense. In-home testing only requires a finger stick, instead of a full blood sample. My former provider also tried this method but was still denied and they will not work with patients who are out-of-network. These out-of-state companies need to file with the BCBS in the state where they are registered.
The replacement of one or more heart valves with an artificial valve. Maintaining a safe PT/INR can help avoid serious complications, such as bleeding or stroke. While regularly checking your PT/INR is important, more frequent testing can help reduce severe risks to your health. Self-testing with the device should not occur more frequently than once a week. Beats having to waste at least an hour round trip driving to the hospital every two weeks.. It was shocking to me how many people I had to randomly talk to at BCBS before I finally, by chance, spoke to someone who immediately knew what the problem was.
Talk to your healthcare provider if you’re interested in pursuing in-home testing. In-home testing is not for everyone, and it does come with challenges. One major issue is that the in-home testing machines can be less accurate than testing at a clinic. Dr. Collins has found that some machines can be up to 20-30% off in measuring INR levels. “It’s not as accurate as a hospital blood draw, but we have additional tools to ensure we’re getting the most accurate result,” said Dr. Collins.
Honestly if I wasn't calling every week at minimum then nothing would have happened. Also each time I called BCBSTX I spoke to a different person and finally happened to talk to someone who knew what the problem was. This should be fairly common with the number of people on blood thinners. I don't understand why this has been such a pain in the ass to get solved. My current provider Acelis Connected Health is 'out of network' causing me to scramble to find a replacement.
A couple of years ago the fda released coaguchek xs for patient home use. If they have someone who can perform the test or are cognitively intact and reliable to do the test themselves and report to md for dosing instructions, they qualify. Medicare pays for it with a doctor rx and so do other insurance companies. This only applies to people on long term coumadin/warfarin therapy such as for heart valve, etc. . How often an INR test meter is used to monitor warfarin, and the results a patient should expect, will be based on recommendations from the patient’s health care provider. It is typically between 2.0 and 3.0 for basic blood-thinning needs, though the range may vary based on a patient’s specific conditions.
When it comes to your overall well-being, maintaining a healthy heart is essential. With Lincare's home INR testing, you have your PT/INR values at your fingertips. Patient self-testing with mdINR®reduces the risk of bleeding and clotting by giving you reliable, real-time results from the comfort of home. These tests save you from a visit to the physician's office or lab to check your PT/INR levels. All health care providers, patients and caregivers, are strongly encouraged to voluntarily report INR test meter problems directly to the FDA through MedWatch, the FDA’s voluntary reporting program. Problems should be reported whenever one suspects there is an issue with an INR test meter such as a malfunction or incorrect result, or that the meter caused or contributed to a serious injury or death.
